What is Medical Respite Care?

Medical respite care provides safe, short-term residential housing and supportive services for individuals experiencing homelessness who are too ill or frail to recover on the streets, but who do not require hospital-level care.

Our program bridges the gap between hospital discharge and stable recovery by offering a safe place to heal, rest, and regain strength while connecting individuals to essential health and social services.

Who We Serve

We support individuals who are:

• Experiencing homelessness or unstable housing
• Recently discharged from a hospital or emergency room
• Recovering from illness, surgery, or injury
• In need of a safe environment to heal and stabilize

Services Provided

• Safe and supportive housing
• 24/7 basic supervision and care coordination
• Medication support and health monitoring
• Connection to primary care and specialty providers
• Case management and discharge planning
• Support with housing placement and long-term stability
